Group personal accident insurance policy covers the persons stated in the policy schedule in event of death, permanent disability and temporary disability caused by violent, accidental, eternal and visible means while on duty or off duty on 24 hour basis. Requires a list of employees/members to be covered on named/unnamed basis, occupations of members, cadres proposed to be covered and cadre strength, coverage sought (Death/Death+PTD/Death +PTD +TTD), TTD cover over 52 weeks/104 weeks, and annual wages, limit of medical expenses cover sought.
