Contractor’s Plant and Machinery

  • Duly completed, signed, and stamped Contractor’s All Risk claim form with detailed statement of loss.
  • Written, signed, and dated statement by Contract Supervisor/ Engineer
  • Project Engineer’s License
  • Police investigation report
  • Copy of contract letter.
  • Damage estimate of contract works.
  • Third party liability letter if any

Contractor’s plant and Machinery

  • Duly completed Contractor’s Plant and Machinery claim form
  • Written and signed statement by the Machine Operator/witness/claimant
  • Copy of Operator’s ’s license
  • Copy of logbook
  • Police inspection report (IOV)
  • Complete police report (where necessary)
  • At least two repair quotation from two different sources
  • Payment receipts / Towing receipts (if applicable)
  • Proof of ownership / Purchase details and agreement
  • Bank signed transfer forms
  • Letter authorizing transfer (Company vehicles)
  • Final bills & purchase receipts of repair where the repair costs are within authorized repair limits

Fidelity Guarantee

  • Filled claim form
  • Details of suspect’s referees
  • Three months bank Statement
  • Statement from the second employee detailing the circumstances of loss.
  • Copy of employment identification for the suspect
  • Copy of employment/appointment letter for the suspect
  • Police report
  • Copy of the dismissal letter for the suspect
  • Details of all benefits due to the suspect
  • Insured’s claim statement showing the amount that was stolen with a proper breakdown

Goods in Transit/Marine

  • Duly completed, signed and stamped Marine / GIT claim form with detailed statement of loss/ damage
  • Copy of policy
  • Written, signed and dated statement by witness / claimant
  • Police investigation report
  • Invoice copy
  • Bill of lading
  • Short landing certificate (if applicable)
  • Packaging list
  • Shipping documents
  • Open delivery certificate; and
  • Copy of letter lodging monetary claim on the carriers.

Money

  • Filled claim form
  • Three months Bank Statement
  • Statement from the eyewitness detailing the circumstances of loss.
  • Copy of employment identification for the employee.
  • Copies of stock purchase details
  • Copies of daily sales register
  • Police report
  • Sales invoices / receipts
  • Insured’s claim statement showing the amount that was stolen with a proper breakdown

Property

  • Duly completed, signed and stamped property claim form with detailed statement of loss
  • Written, signed and dated statement by witness / claimant
  • Police / Fire brigade investigation report
  • For stock, purchase invoices/ proforma invoices/ receipts of stolen or damaged property
  • At least two sourced quotation for stolen / damaged items from different providers
  • At least three repair quotations for damaged premises / property from different providers
  • Proof of ownership / purchase details and agreement
  • Internal Auditor’s report / accounts documents/ stock taking report before and after incident
  • Third party liability letter if any

Liability

  • Duly completed Liability claim form
  • Incident Report
  • Police Report
  • Colour Photos of damaged property and location
  • CCTV footage showing circumstances of incident
  • Assessment Report from repairer on the cause and extent of the property damage
  • At least 2 Repair / Replacement Quotations
  • Purchase invoice/ receipt of damaged property
  • Tenancy and/or Contract Agreement with third party contractors (where applicable)
  • Third party claimant’s letter of intention to claim, Writ of Summons, etc.
  • Final bills & purchase receipts of repair where the repair costs are within authorized repair limits

Workman’s Compensation

  • Duly completed claim form
  • LD Forms (31, 35, 87 as applicable) in case of incapacitation)
  • Appointment letter
  • Copy of ID
  • Supervisor’s report
  • Complete original medical reports / payment receipts/cash memos and other expenses, including admission, discharge summary, x-ray report, scan blood report (Where necessary)
  • Sick leave certificate (If awarded any)
  • Copy of pay slips for 3months prior to accident and one month after accident

Personal Accident

  • Duly completed claim form
  • Appointment letter
  • Supervisor / witness’ report
  • Complete police report (where applicable)
  • Complete original medical reports / payment receipts/cash memos and other expenses, including admission, discharge summary, x-ray report, scan blood report (Where necessary)
  • Sick leave certificate
  • Copy of pay slips for 3months prior to accident and one month after accident
